Technical Assistance:
Discussed with Provider that anyone left alone with Children are required to have the necessary paperwork required by Licensing.

Comments:
Today an unannounced monitoring visit was conducted with a Caregiver and Provider. This visit took place in the morning between 11:00am-12:30 there were 3 children for a total of 12 points within the supervision guidelines. The children were observed having free play, taking nap. A complete inspection of the physical plant, children and provider records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-120-B
Description: Based on review of five staff records, the provider did not maintain complete staff records. Evidence: Staff #1's record, did not include documentation of age verification, name, address and phone number, references, emergency contact person.

Plan of Correction: Staff stated that the provider had an emergency. Provider that she will complete a file for this person.

Standard #: 22VAC40-111-120-C
Description: Based on review of provider records, the substitute provider record did not include documentation of time of arrival and departure. Evidence: The substitute provider form did not include the name of the person who was left alone with the children.

Plan of Correction: Provider will make sure to document times.

Standard #: 22VAC40-111-150-D-1
Description: Based on documentation review on-site and interview, the assistant who was left along with children did not obtained first aid and cardiopulmonary resuscitation (CPR) certification. Evidence: Staff #1 who was alone with children upon arrival did not have documentation of First aid & CPR.

Plan of Correction: Caregiver stated that she has completed first aid & Cpr but did not have certificates. Provider stated that she made sure she had first aid and CPr prior to leaving the children. Provider will fax a copy.

Standard #: 22VAC40-111-170-A
Description: Based on review of caregiver record, the caregiver did not have documentation of a tuberculosis (TB) test or screening. Evidence: Caregiver/Staff #1 did not have TB TEST on-site.

Plan of Correction: Provider stated that she had it but just left a copy at home.

Standard #: 22VAC40-111-200-A
Description: Based on review of five staff records, the provider did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Upon arrival, Caregiver #1 did not have documentation of orientation training.

Plan of Correction: Provider will make sure she has orientation training.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on review of one staff records, the provider did not have a completed sworn disclosure statement for each staff prior to date of hire. Evidence: Upon arrival, staff #1 who was alone with the children did not have a completed Sworn Disclosure Statement.

Plan of Correction: Completed during this inspection.
