An announced initial study was conducted today from 9:30am to 11:10am. 3 children (9points) and 2 caregivers were present. Ratios were in compliance. The physical space, caregiver records, household member records, children's records, the Emergency Evacuation Plan, evacuation drills, emergency supplies, attendance sheets, the menu and the evacuation map were reviewed. Required documents were posted. The space was clean and organized. An ample supply of age appropriate materials were available for the children to play with. Standard #: 22VAC40-111-100-A Description: Based on review of 3 children's records, the provider did not obtain documentation of a physical examination within 30 days after the first date of attendance. Evidence: Child #1 started on 3/20/2017, Child #2 started on 10/21/2017 and Child #3 started on 10/10/2017. All three children had immunization records but no health form with evidence of a physical examination. Plan of Correction: I will ask all of the parents to obtain the needed documentation. (Provider printed out the Health Form while LI was present so parents would have the form this afternoon.)
Standard #: 22VAC40-111-60-B Description: Based on review of three children?s records, the provider did not maintain complete children?s records. Evidence: Parent's addresses were missing for Child #1 & Child #2. Work addresses and/or work phone numbers, Emergency contact addresses and Medical Insurance Policy Numbers were missing for Child #1, #2 & #3. A signed Liability Insurance Declaration was missing for Child #2. Plan of Correction: We will ask the parents for the information right away.
