The licensing inspector conducted an unannounced monitoring inspection from 8:55am - 11:30am. Upon arrival, seven children were in care for a total of 21 points with one caregiver. An additional caregiver arrived at approximately 9:00am and an additional child came into care at 9:42am. The children were observed playing with blocks, playing with dress up clothes, during snacks and listening to a story. Two caregiver's records and five children's records were reviewed. Standard #: 22VAC40-111-520-A
Description: Based on observation, the family day home failed to have cribs for each infant in care. Evidence: 1. The inspector observed the available cribs. The family day home had three infants in care, but only had two available cribs.

Plan of Correction: The provider ordered an additional crib during the inspection. The crib is scheduled to arrive on Friday, 12/15/2017.

Standard #: 22VAC40-111-570-A
Description: Based on observation, record reviews and interview, the family day home failed to have an additional caregiver when the total points for children in care exceeded 16. Evidence: 1. Upon arrival at 8:55am, the inspector observed seven children in care for a total of 21 points with one caregiver. 2. The inspector reviewed the attendance sheet and all seven children had been in care for at least 45 minutes with one caregiver. The inspector also reviewed the date of birth for all seven children to determine points. 3. The inspector interviewed the provider. The provider said the assistant usually was at the family day home at 8:30am.

Plan of Correction: Two children arrived earlier than expected today. The provider will ask the parents to let her know ahead of time when the children will arrive earlier than expected so her assistant can come in earlier.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record reviews, the family day home failed to obtain repeat criminal history record checks. Evidence: 1. The inspector reviewed two caregiver's records. Caregiver record #1 and caregiver record #2 contained criminal history record checks that were over three years old. Both checks were dated 10/17/2014.

Plan of Correction: The provider has already requested repeat checks for both caregivers, but to date has not received the checks. When the checks are obtained the provider will send the licensing office a copy of both checks.
