-New background check requirements were reviewed with the provider.

An unannounced renewal inspection was conducted on 12/20/2017 from 10:50 am - 1:15pm. At the time of entrance there were seven children in care with two caregivers. The ages of the children counted for a total of 18 points. Children were observed during morning program time which include outdoor play, art, reading and music. Lunch and nap were additionally reviewed. The sample size was seven children's records, two caregiver records and one household member. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Standard #: 22VAC40-111-100-B
Description: Based on record review and interview, the provider failed to obtain a physical examination prior to attendance or within 30 days of attendance. Evidence: 1. There is no documented physical exam on file for child 1 who was in care during the inspection. Child 1 has an enrollment date of 08/21/2017. 2. The provider confirmed that the physical exam was not on file for child 1.

Plan of Correction: A physical was obtained for child 1 however, it has been misplaced. The parent has been contacted and is bringing a copy of the physical exam later today.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview, the provider failed to obtain for an adult household member a Tuberculosis Screening every two years from the date of the first screening. Evidence: 1. The TB screening for one household member was last conducted on 10/02/2015. 2. The provider verified this at the most current TB screening.

Plan of Correction: An updated TB screening will be obtained.
