Comments:
A Subsidy Health and Safety inspection was conducted on 12/21/2017 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 9:40am-11:20am. There were 7 children in care (18 points) directly supervised by 2 caregivers. A sample of 4 children?s, 2 caregivers records and 2 household member's records were reviewed. The children were observed playing outside, watching television, eating yogurt and playing with toys. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDHOM-000-005-B
Description: Based on observation, the daily departure for each child was not recorded. Evidence: The arrival for each child was recorded on the daily attendance sheet. The daily departure for each child was not recorded.

Plan of Correction: I will write in and out for each child.

Standard #: VENDHOM-000-012-C
Description: Based on record review and interview with provider, all caregivers hired did not complete the Virginia Pre-Service training. Evidence: Provider stated that Caregiver A hired 7/11/2017, had not completed the Virginia Pre-Service training.

Plan of Correction: She will complete it.
