Comments:
A Subsidy Health and Safety inspection was conducted on 01/08/2018 to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on review, all caregivers hired prior to the effective date of this regulation, did not complete Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services within 90 days of the date this regulation became effective. Evidence: 1. The Assistant has been employed since 06/2015 and did not have documentation that she has completed the Virginia Pre-Service Training.

Plan of Correction: I will ask my assistant to complete the training.
