- Technical Assistance:
Discussed the recent Code changes pertaining to background checks and the upcoming fingerprint requirement for all staff and household members.
An unannounced monitoring inspection was conducted this date. A total of 11 children (42 pts) with Provider and 2 assistants present during the inspection. Walk thru of programming conducted to include the crib room. The home provides ample toys and equipment determined appropriate for the ages of the children currently in care. Children were observed free playing, dancing to music and being fed. Handwashing and diapering procedures observed today. Lunch was served and the children sat with staff and ate. Ample amounts were available for second helpings if the children requested. A total of 7 children's records were reviewed, all staff(3) records and 2 household background checks were reviewed. No medications on site as the provider does not administer medications. Some violations were found today and a corrective action was provided. The previous violation from the April inspection was reviewed and corrective action was verified. Thank you for your cooperation today. This inspection began at 11:05am and ended at 1:15pm. Standard #: 22VAC40-111-120-B Description: Based on staff record review, one record was found not complete. Evidence: Staff #3's record did not include the start date of employment. Plan of Correction: This staff began on 12/11/17. The record will be updated with the start date.
Standard #: 22VAC40-111-180-A Description: Based on staff record review, subsequent TB screening was not obtained. Evidence: Staff #2's most recent TB screening on file is dated 9/21/15. A TB screening must be obtained every two years. Plan of Correction: This staff will obtain an updated TB screening.
Standard #: 22VAC40-111-210-A Description: Based on staff record review, annual staff training was not obtained. Evidence: Staff #1 and #2 has no documentation of 16 hours of annual training. Most recent staff training is dated in 2015. Plan of Correction: We will all obtain the required training beginning in January
Standard #: 22VAC40-111-280 Description: Based on observation of the physical plant, hazardous substances were found unlocked. Evidence: Cleaning agents were found in an unlocked cabinet under the sink in the programming area. This area is accessible to the children in care. Plan of Correction: We will either move these cleaning products or place a child lock on the cabinet.
Standard #: 22VAC40-111-530-B Description: Based on review of the crib room, bottom covers were not appropriate. Evidence: The crib sheets were not tight-fitting. Plan of Correction: We will request the parents to bring new sheets that fit tighter.
Standard #: 22VAC40-191-60-B Description: Based on staff record review, a Sworn Statement was not found for one staff. Evidence: Staff #3 began employment on 12/11/17. No documentation found that a Sworn Statement was completed. Plan of Correction: This staff will complete the Sworn Statement today.
