An unannounced renewal inspection was conducted today, January 10, 2018 to determine the provider's compliance with licensing standards. The inspection was initiated at 1:40pm and concluded at approximately 4:15pm. In attendance were 4 children totaling 14 points being supervised by the provider. Children were initially asleep, then later used the restroom, and prepared for snack consisting of pineapples, mini muffins, and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. Hazardous chemicals were found in a locked closet and sharp utensils inaccessible to children. All rooms of the home were inspected with no hazards found. The family day home had the required postings displayed on the wall upon entering the kitchen. The first aid kit, fire extinguisher, and emergency supplies were inspected. Two children's records and provider records were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, daily attendance, and other pertinent information. Description: Based on the inspection of administrative records the required information was not listed. Evidence: During the inspection of two children's records, it was discovered that child #1 did not have a second emergency contact listed in the record. Licensing standards require two emergency contacts to be listed with the full address and telephone number for each emergency contact.

Plan of Correction: Per provider: I will have the parent to list a second emergency contact by the end of the week.
