Discussed that the weekly menu needs to be posted.
An unnanounced renewal inspection was conducted from 10:40 am - 1:00 pm. There were 11 children present, ranging in age from 15 months to four years of age, with the provider and two assistants. The children were observed playing outside, listening to stories and napping. Five children's records, three staff records, and one household member's record was reviewed. Standard #: 22VAC40-111-120-B Description: Based on a review of three staff records, not all contained documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Staff A, date of hire 1/8/2018, did not have documentation of two reference checks. Plan of Correction: The staff member forgot to bring them in. Will ask staff member to bring in the references tomorrow.
Standard #: 22VAC40-111-90-C Description: Based on a review of five children's records, it was determined that not all immunizations for children under two were updated every six months. Evidence: Child A's (date of birth 7/23/16) last immunizations were dated 9/23/16. Plan of Correction: Will contact child's parents to bring in updated immunization record.
Standard #: 22VAC40-111-280 Description: Based on observation, not all poisonous substances were stored in a location inaccessible to children. Evidence: Lysol, Chlorox wipes, and bleach water were stored on a high, open shelf. GermX, with a label stating "keep out of reach of children," and "flammable" was also observed on an open shelf. Plan of Correction: Poisonous substances were moved to a locked storage room during the inspection.
