Technical Assistance:
During the inspection, licensing representative discussed with provider diapering, restroom supplies, required postings, updates to the national fingerprinting/background check procedures, and requirements for potential assistants/substitute providers. Provider reported that they are no longer a subsidy vendor and have made attempts to return vendor card machine.

Comments:
The following inspection took place between the hours of 1:00pm through 2:55pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there was 1 child in care (3 points) with 1 caregiver. 1 child's record, 4 caregiver/household member's files were reviewed, in addition to emergency supplies, drills and plans. Positive and nurturing interactions were observed between caregiver and child. Inspector observed child engaged in toy play and games (hide and seek) with the Provider. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain the required supplies. Evidence: The first aid kit was missing 1 of 2 triangular bandages and first aid instructional manual.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-60-B
Description: Based on a review of records, 1 child's record did not contain the required information. Evidence: The record for Child #1 [date of enrollment: 1/16/2018) was missing 1 of 2 required emergency contacts, and parents' employer's address and phone number.

Plan of Correction: I will ask the parents for the information. If they do not want to disclose the information I will make a note of it.

Standard #: 22VAC40-111-90-A
Description: Based on a review of records, 1 child's record was missing documentation of immunizations prior to the first day of attendance. Evidence: The record for Child #1 (date of enrollment: 1/16/2018) was missing documentation of immunizations.

Plan of Correction: The parent has requested this information from the child's physician. It will take a few days for them to send it to me.

Standard #: 22VAC40-111-210-A
Description: Based on a review of records, documentation of the required 16 hours of annual training was not available at the time of inspection for one caregiver. Evidence: The record for Caregiver #1 included documentation of only 10 out of the required 16 hours of annual training and development needed during the 2017 calendar year.

Plan of Correction: I did not have children in care for several months and did not continue training. I have trainings scheduled for the end of this month that I intend to complete.
