An unannounced monitoring inspection was conducted today from approximately 11:20am-1:40pm. There were 11 children (24 points) directly supervised by 3 caregivers. A sample of 6 children?s records, 2 caregiver's records and 1 household member's were reviewed. The children were observed playing, eating lunch, and napping. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Description: Based on observation, the emergency plan was not reviewed annually. Evidence: The emergency plan was last reviewed 3/7/2014.

Standard #: 22VAC40-111-830-C
Description: Based on record review and interview, the evacuation drills for December were not documented. Evidence: The provider stated that an evacuation drill was conducted on December 11th 2017. This drill was not recorded on the evacuation drill document.

Description: Based on records reviewed, the children's records were incomplete. Evidence: 6 of 6 children's records reviewed were incomplete. 1) Child A was missing an annual update, file last updated 4/8/16. 2) Child B was missing an annual update, file last updated 8/16/16. 3) Child C was missing an annual update, file last updated 2/24/26. 4) Child D was missing an annual update, file last updated 6/9/15. 5) Child E was missing an annual update, file last updated 2/24/16 6) Child F was missing proof of identification before the first day. Date of entrance 1/2/2018.

Description: Based on observation, there was no record of arrival and departure times for each day that the substitute provider worked. Evidence: During the last inspection which occurred August 2017, the Provider was out of the country and the substitute provider was running the facility. There is no documentation of the hours that the substitute provider worker during the time the provider was out of the country.

Description: Based on observation, electrical outlets were not covered with protective covers. Evidence: 2 unused electrical outlets near the sofa in the basement (main play area) were not covered with protective covers.

Description: Based on observation, the bathroom did not contain the required contents. Evidence: The bathroom in the basement did not have toilet paper.

Description: Based on observation, the diapering table was not nonabsorbent. Evidence: There were 2 tears on the nonabsorbent surface which revealed the cotton like fiber underneath the nonabsorbent fabric making the surface absorbent in those areas.

Description: Based on observation, the most recent violation notice was not posted. Evidence: The most recent inspection was conducted August/2017 and the violation notice posted was from an inspection conducted February/2017.
