Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on January 24, 2018 from 10:50 a.m. through 12 :50 p.m. Upon arrival, there were twelve (12) children in the care of the provider and one assistant. Six (6) children's records were reviewed and updated. The records for the provider, one assistant and one household member were also reviewed and update. Areas of non-compliance may be found on the violation notice. Areas observed were: lunch, hand washing/bathroom and diapering procedures, story, a short movie and nap. Interactions between the provider and children were observed as positive and the children seem comfortable with the daily routine and schedule. Description: Based on record review an updated tuberculosis screening was no obtained for the provider and one household member. Evidence: 1. Last documented tuberculosis screening for Household Member A was dated 06/18/2015. 2. Last documented tuberculosis screening for the provider was dated 06/15/2015. 3. The provider confirmed updated tuberculosis screenings had not been obtained.

Plan of Correction: I will obtain an updated tuberculosis screening for myself and household member.

Description: Based on record review, the provider failed to obtain an updated central registry search of one staff member. Evidence: 1. Staff #1 had not obtained an updated central registry search. 2. Last documented central registry search for Staff #1 was dated 02/14/2017. 3. The provider confirmed an updated central registry search had not been completed for Staff #1.

Plan of Correction: I will complete an undated central registry search for Staff #1 immediately. Documentation will be sent to the licensing inspector.
