Comments:
An unannounced monitoring visit was conducted today between the hours of 12:04pm through 3:00pm with the provider on site. There were 13 children (36 points) with the provider and her assistant. A sample of 5 children's files and 2 staff member's files were reviewed. The children were engaged in the following: toy play, lunch, infant care, and nap time. Violations:
Standard #: 22VAC40-111-730-C
Description: Based on observation, the provider stored medications in a refrigerator used for food and did not store the medication in a container or in a clearly defined area away from food. Evidence: 1. The medication was on a shelf in the refrigerator along with baby bottles and cups and was not in a container or in an area away from food.

Plan of Correction: I will look for some type of container to put it in.

Standard #: 22VAC40-111-730-D
Description: Based on observation, the provider did not keep medication in a locked place using a safe locking method that prevents access by children. Evidence: 1. The provider had a prescription medication on site for Child E that was kept unlocked in a small refrigerator in the room the children use.

Plan of Correction: I will look for some type of container and use a lock on it.

Standard #: 22VAC40-111-30-A
Description: Based on observation, the provider did not ensure compliance with the current license issued by the department. Evidence: 1. The current license issued by the department has an approved capacity of 12, however during today's inspection there were 13 children present.

Plan of Correction: When school is out, I will not allow the school age children to come. I will hire an assistant.

Standard #: 22VAC40-111-60-B
Description: Based on review, two of five children's records were not complete. Evidence: 1. Child A was missing one parent's work number. 2. Child B was missing one parent's work address.

Plan of Correction: I will ask for the information.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas and furnishings of the family day home inside were not maintained in a safe, and operable condition. Evidence: 1. In the room where the children play, a blue chair that children sit on or climb on has a very large rip in the cloth that covers the chair.

Plan of Correction: I will throw it away.

Standard #: 22VAC40-111-570-A
Description: Based upon observations made and documentation reviewed, the provider did not ensure that a caregiver did not exceed 16 points. Evidence: 1. Upon arrival to the family day home, there were 13 children (36 points) under the supervision of 2 caregivers, exceeding the required 16 points per caregiver by 4 points. There were 4 children ages infant to 15 months, 3 children ages 16 months to 23 months, 5 children ages 2 years to 4 years and 1 child age 5 years old.

Plan of Correction: I am in the process of hiring an assistant.

Standard #: 22VAC40-111-600-B
Description: Based on observation, two Infants spent more than 30 minutes of consecutive time during waking hours confined in a crib, play pen, high chair or other confining piece of equipment. Evidence: 1. Child A was confined to a bouncy seat from approximately 12:05pm to 12:44pm, was offered a bottle for 2 minutes and then put back in the bouncy seat to play. The provider picked him up at 1:11pm. 2. Child F was confined to a bouncy seat next to child A from approximately 12:05pm to 12:46pm and was then moved to a standing bouncy seat.

Plan of Correction: I usually do.
