Comments:
A Subsidy Health and Safety inspection was conducted on January 18,2018 to review supplemental health and safety requirements. There were 7 children with the Provider and 2 assistants for a total of 24 points within the supervision guidelines. The children were observed having morning free play, puzzles, singing songs, lunch and nap time. Positive interactions were observed during this inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDHOM-000-029-B-5-F
Description: Based on review of the emergency plan, Provider did not have procedures in place to reunite children with parent in the event of an emergency. Evidence: Emergency plan did not include procedures for reunification.

Plan of Correction: Provider stated that she will add to the emergency plan.
