Discussed with Provider that Background checks/fingerprint check should be conducted immediately and staff are not allowed to work in the family day home until it is completed.
Today an unannounced monitoring visit was conducted between 9:00am-11:15 with the Provider there were 8 children with the Provider and 2 assistants for a total of 24 points within the supervision guidelines. The children were observed having morning nap time, outdoor free play, lunch and nap. Positive interactions were observed. A complete inspection of the physical plant, children and provider records, fire drill log, and emergency procedures were observed. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles were found without name and date. Evidence: Upon arrival, three infants were present however 2 bottles were found without the name and date. Plan of Correction: Provider stated that when they wash the bottles the labels and dates come off.
Standard #: 22VAC40-111-530-F Description: Based on observation, soft bedding was found under an infant who was in a crib. Evidence: Upon arrival, a 7 month old was in a crib with soft blanket under them while in the crib Plan of Correction: Provider immediately removed it during this inspection.
Standard #: 22VAC40-191-40-D-1-b Description: Based on review of the provider records, one new caregiver record and interview with the provider, the provider did not obtain a central registry report and criminal record within 30 days of hire. Evidence: Upon arrival, Caregiver #1's record did not include documentation of a Criminal Record check and central registry check before 30 days of employement ends 9/17. Plan of Correction: Provider stated that she submitted the forms in December and they should be arriving any day.
