- Technical Assistance:
Finger print registry consultation was given on the new background check requirements. There is a revised Sworn Disclosure Statement and Provider needs to establish a point of contact for a facility code to use on finger prints. Please direct all questions regarding finger prints to OBI 804-726-7884 Immunization records are due on the day a child starts and 30 days allowed for current physical examination report.
An unannounced inspection was conducted today. There were 7 children present and 6 were sleeping. The Provider was alone with the children for a total of 18 points. The assistant was outside the home and when he returned was responsible for a family member so was unable to help in the ratio. The Provider had a new enrollment and all paperwork was maintained with the exception of the Immunization record. Please call with questions 703-479-4679
Standard #: 22VAC40-111-210-A Description: Based on record review of assistant, the assistant did not obtain all 16 hours of training in the last year. Evidence: 1. Training documents were reviewed for the Provider and assistant and the documents verify 12 hours have been completed. 2. The annual training requirements require 16 hours of training. Standard #: 22VAC40-111-320-G Description: Based on interview, the Provider has not obtained an annual wood burning stove/fireplace inspection. Evidence: 1. Inspection was conducted over a year ago and Provider stated she has not had another one conducted. Plan of Correction: I will call this week and whenever they can come out.
Standard #: 22VAC40-111-570-A Description: Based on observation of the children and record review, it was determined the Provider exceeded the 16 points per caregiver. Evidence: 1. Inspector arrived at nap and the Provider was alone with 7 children. Record review was conducted to determine ages of children in care and the 7 totaled 18 points. Plan of Correction: I have a child leaving this week and a new child started today. From now on I will make sure not to go over.
