Technical Assistance:
Reviewed the timing for the completion of all required fingerprint background checks. Discussed how to accurately document each month's emergency evacuation drill.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 3 children, totaling 6 points. Children's records, adult records, caregiver training, children's nutrition, emergency preparedness items, and the physical plant were inspected today. It was reported to the Inspector that no children's medications were on site today. The children were observed playing with toys, engaging in creative play with stuffed animals, assembling a train set, and completing academic work. This inspection was completed between approximately 10:00am and 11:20am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1does not include: the information for a second emergency contact person. 2. Child record #2 does not include: the information for a second emergency contact person; the provider's signature indicating that she has viewed the child's original proof of identification; the parent's signature authorizing the provide to obtain emergency medical care if needed.

Plan of Correction: The parents will be reminded that I need the information today. I saw the birth certificate but forgot to document it. I will document it today.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, it was determined that two files do not include documentation showing that the parents have been given all required enrollment information in writing. Evidence: Child record #1 and #2 do not include a signed "Information for Parents" document. The provider has an enrollment handbook, but this handbook does not include all information that is is included on the Information for Parents document.

Plan of Correction: I gave the form to the parents. I will remind them to sign and return it at drop off tomorrow.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that one electrical outlet was not covered. Evidence: One electrical outlet next to the children's work table area was not covered.

Plan of Correction: The outlet was immediately covered during the inspection.
