Comments:
An unannounced renewal inspection was conducted today from 10:45 am - 12:30 pm. There were 7 children present with the provider and assistant for fifteen points. I observed children active, riding a choo-choo train in the playroom; working with puzzles; crafting a police car with the current transportation unit and eating lunch. Records were well-organized and a wide supply of activities and materials were available. If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's files today, some information was missing. Evidence: There was no policy number for health insurance for Child # 1 and Child #2. There was no address for an emergency contact for Child # 2.

Plan of Correction: I will talk to the parents today.

Standard #: 22VAC40-111-230-B
Description: Based on a review of records today, there were no documents available for annual training for Assistant # 1. Evidence: Assistant # 1 reported that 20 hours of training was recently completed, but no certificates were available for review today.

Plan of Correction: She will bring in the certificates.
