Today an unannounced Monitoring inspection took place between 9:45am and 11:30am. There were 8 children present with the provider and 1 assistant for a total of 26 points within the supervision guidelines. The children were observed having tummy time, playing freely, washing hands and starting to eat lunch. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-800-B Description: Based on record review, the emergency preparedness plan is not reviewed annually as required. Evidence: There is no documentation that the child's emergency preparedness plan has been reviewed since it's inception on 5/1/2015. Plan of Correction: I'll update that.
Standard #: 22VAC40-111-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain addresses or phone numbers for both emergency contacts, the child's medical insurance company name or policy number. Child #3 file did not contain the child's first date of attendance. Child #4 file did not contain the child's medical insurance policy number. Child #5 file did not contain a phone number for their first emergency contact. Plan of Correction: I'll return the children's files back to the parents and they will not be able to return until all the information is filled in completely.
Standard #: 22VAC40-111-210-A Description: Based on record review, the provider has not obtained the required 16 hours of annual training. Evidence: The provider has completed 1 hour of training since 1/30/2017. Plan of Correction: I've completed at least 4 or 5 online credits through the VA online program but I have not printed the certificates. I am registered for some coming up in the future.
