Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on February 6, 2018 from 11:30 a.m. through 3:30 p.m. Upon arrival, there were nine (9) children in the care of the provider and two (2) assistants with points totaling thirty-two (32). Five (5) children's and the records for the provider, one household member and three (3) assistants were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were: free choice activity, lunch, hand washing/diapering procedures and bathroom procedures and nap. Interactions between the staff and children were observed as positive and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-111-60-B Description: Based on record review, one child's record did not contain the physical address for two emergency contacts. Evidence: 1. Five children's records were reviewed. 2. Child #4 did not have documentation of a physical address for two emergency contacts. 3. The provider confirmed the physical addresses for Child #4 were not documented. Plan of Correction: I will ask the parent to provide the physical address for the two emergency contacts.
Standard #: 22VAC40-111-680-B Description: Based on observation, three children did not wash their hands after being diapered. Evidence: 1. Three children were observed during a diaper change. 2. Staff # 1 failed to wash the hands of all three children after being diapered. 3. Staff #1 confirmed she did not wash the children's hands after changing their diapers. Plan of Correction: I will review all hand washing procedures will all of my staff.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering changing pad was torn and the inside padding was exposed. Evidence: 1. The diaper changing area was observed and inspected. 2. The nonabsorbent pad was torn and the inside padding was exposed. 3. The provider confirmed the diaper pad was torn. Based on observation, staff failed to clean and sanitized the diaper pad after changing three children's diapers. Evidence: 1. Diaper changing was observed for three children. 2. Staff #1 did not clean and sanitized the diaper pad after changing each child. Staff #1 did wipe the changing pad with a disposable wipe after changing each child. 3. Staff #1 confirmed she did not clean and sanitized the diaper changing pad. Plan of Correction: I will purchase a new nonabsorbent diaper pad as soon as possible. Staff will be reminded that the changing table is to be cleaned and sanitized after changing every child.
