Today's visit was a routine monitoring inspection. There were five children present with one caregiver and a total of eleven points. The inspection began at 9:45am and concluded at 12:15pm. During the inspection infant care was observed, children were observed playing with blocks, hand washing was observed, lunch time was observed, nap time was observed, all six children's records were reviewed, the licensee's record was reviewed, an assistant/ spouse's record was reviewed, emergency supplies were viewed, and some policies and procedures were reviewed. The provider stated she does not provide any transportation and does use the outdoor swimming pool for children in care. Discussion took place regarding new background check requirements. Standard #: 22VAC40-111-780-B
Description: Based on the inspector's review of all six children's records, the provider failed to ensure a record had documentation of the parent's annual review of the emergency contact information. Evidence: The record for child #1 has not had documentation of the parent's review of emergency contact information.

Plan of Correction: The provider will have the parent review and document the review of the child's emergency information at the time of pick up.

Standard #: 22VAC40-111-110-B
Description: Based on the inspector's review of all six children's records, the provider failed to ensure an immunization update for a child was signed or stamped by a physician, his designee, or a health department official. Evidence: The record for child #5 has a print out of an updated immunization record dated 11/10/17. The record is not signed in any manner by a physician, his designee, or a health department official.

Plan of Correction: The provider will have the parent provide a signed copy of the child's updated immunization record
