Comments:
An unannounced renewal monitoring inspection was conducted from 12:25pm to 1:00pm. At the time of inspector's arrival there were 5 children, ages 1 to 4 years, for a total of 13 points, in care with the provider. The records sample size consisted of 5 children's records and 3 provider / assistant/ household member records. Children were observed interacting with the provider, diapering, handwashing, and napping. The provider stated that currently no medication is administered to children while in her care. The provider also reported there were no incident reports since the last inspection. The area of noncompliance is detailed in the violation notice and was discussed with the provider during this inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review it was determined the provider did not ensure that each chid's record contained all of the required information. Evidence: The record for child #1(enrolled 10/30/17) did not contain the medical insurance policy number for the child.

Plan of Correction: Provider will request this information from parent at pick up.
