02/16/2018 Arrived 8:15am Departed 10:50 am An unannounced renewal inspection was conducted today. Three children were in care for a total of 6 points and were working puzzles, playing with pretend food and reading books. Five children's files and three adult files were reviewed. Standard #: 22VAC40-111-240-A
Description: Based on a tour of the home, the facility failed to insure that all areas are safe and clean. Evidence: 1. There is an ashtray containing cigarette butts on a chair beside the front door.

Plan of Correction: The ashtray will be moved to the top of the grill.

Standard #: 22VAC40-111-320-C
Description: Based on a tour of the home, the facility failed to insure that there are no electrical devices accessible to children that could come into contact with water. Evidence: 1. There was an electric candle wax warmer on the bathroom sink that was plugged in and was accessible to children.

Plan of Correction: The provider immediately removed the candle wax warmer.
