Comments:
A Subsidy Health and Safety inspection was conducted on 2/20/2018 to review supplemental health and safety requirements. At the start of this unannounced monitoring inspection there were a total of 8 children (18 points) with 2 caregivers. A total of 5 children's files and 4 staff/household member's files were reviewed. The children were observed during bathroom breaks, hand washing, and other organized activities. Chicken nuggets, pasta, oranges and corn were served for lunch today. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on a review of records and discussion, lockdown procedures were not practiced annually. Evidence: Caregiver #1 reported that a lockdown procedure was not practiced within a year of signing new vendor agreement.

Plan of Correction: I will practice a lockdown drill.
