Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and board officers must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. Based on the information on file, the Family Day Home (FDH) will be able to start their fingerprint checks on May 1, 2018. If you did not receive your Facility ID number, your Field Print code numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884.

The following inspection took place between the hours of 9:50 am through 12 pm. Today, I met with the caregivers on site at this Family Day Home (FDH) located in Sterling, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children (29 points) (2 were residents) with 2 caregivers. The Provider was away on an appointment. A total of 5 children?s files, 3 staff files, and 5 household member?s files were reviewed. The children were engaged in the following: infant feeding, infant napping, games, toy play, lunch, and other organized activities. If you should have any future questions, please call. Thank you.

Standard #: 22VAC40-111-60-B
Description: Based on review, 2 of 9 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (02/23/2018), 1 of 2 completed emergency contact information was listed for Child #1. 2. The immunization records for Child #9 (as required by 22 VAC 40-111-90), and the results of the health examination also for Child #9 (as required by 22 VAC 40-111-100). The start date for Child #9 was listed as 11/03/2017.

Plan of Correction: All missing information/documentation shall be obtained and placed on file for each applicable child.

Standard #: 22VAC40-111-460-C
Description: Based on review, the pet on site at the Family Day Home (FDH) had a vaccination that has expired. Evidence - 1. On the date of inspection (02/23/2018), the rabies vaccination for "Mater," the family dog was listed as expired as of 01/15/18. 2. This pet was observed in the home on the date of inspection.

Plan of Correction: This record shall be updated.

Standard #: 22VAC40-111-570-A
Description: Based on observation and interview, the Family Day Home (FDH) did not ensure that the ratio would exceed 16 points per caregiver. Evidence - 1. On the date of inspection (02/23/2018), from the start of the inspection at 9:50 am through 10:05 am there were 9 children on site (29 points) with 1 caregiver (Staff #3). The children consisted of 7 children in care and 2 children that were residents. 2. Staff #2 arrived at 10:05 am, stating that she had over slept. 3. Staff #2 also stated (during an interview conducted on the date of inspection) that the Provider was away on an appointment.

Plan of Correction: Staff will arrive on time, and the point ratios shall be in compliance to where no one staff shall have more than 16 points.
