An unannounced initial inspection was conducted February 16 2018 between 9:30am and 12:00pm. There was 1 child in the direct care of 2 providers. The child was observed working on school work and watching a television program. The home is clean, organized, and offers plenty of space for children to utilized. Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 1 child records reviewed did not have the required information Evidence: Child #1 record did not contain documentation of a 2nd emergency contact complete with address and telephone number.

Plan of Correction: I will have the parent fill in the information at pick up time.

Standard #: 22VAC40-191-40-C-1-a
Description: Based on record review, the applicant's record does not have a sworn statement or affirmation Evidence: The applicant did not have a filled out sworn statement at the beginning of the inspection.

Plan of Correction: I filled it out during the inspection.

Standard #: 22VAC40-191-40-C-1-b
Description: Based on record review, 1 staff member and 1 household member do not have the required background checks. Evidence: Staff #2 does not have documentation of a completed Central Registry search, Criminal Record search, or a Sworn statement. Household member #1 does not have documentation of a sworn statement.

Plan of Correction: I will have the household member sign it this evening. I will call the agency to see if they received Staff #2 background checks.
