An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 11 children, totaling 33 points. Children's records, caregiver records, training records, administrative records, health records, emergency preparedness items, and the physical plant were inspected today. The children were observed engaging in creative play activities, listening to stories, assembling puzzles, and preparing for an afternoon birthday party. The children were to be served rice, beans, fruit salad, and milk today. This inspection was completed between approximately 10:00am and 11:40am. Thank you for meeting with me today. Standard #: 22VAC40-111-920
Description: Based on a review of posted documents, it was determined that the menu was not dated correctly. Evidence: The posted menu was for the month of February.

Plan of Correction: My power went out last weekend and I haven't had time to print the March menu since the power came back on. I will print the menu today.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that the provider did not obtain a physical exam report for two children within 30 days after their first day of attendance. Evidence: Child record #1 and #5 do not include a physical exam report. Child #1 entered care on 2/6/17; child #5 entered care on 1/22/18.

Plan of Correction: I will remind each parent to return their child's physical exam report.

Standard #: 22VAC40-111-60-B
Description: Based on children's records, it was determined that one record was not complete. Evidence: Child record #1 does not include the parent's medical insurance policy number.

Plan of Correction: I will ask the parent for the information today.
