Description: Based on review of emergency supplies, the first aid kit was incomplete. Evidence: The first aid kit was missing two required triangular bandages.

Plan of Correction: I will buy 2 triangular bandages.

Standard #: 22VAC40-111-70-A
Description: Based on child record review, 1 of 5 children's records did not have documentation of parents being provided required written information before their child's first day of attendance. Evidence: Child#4's record is missing documentation related to the home's medication policy and liability insurance.

Plan of Correction: I will have the parents review and sign the form today.

Standard #: 22VAC40-111-90-C
Description: Based on review of 5 children's records, the provider did not obtain documentation of updated immunizations once every six months for children under the age of two years. Evidence: Child#1 has an immunization record on file that was last updated on 4/17/17. Child#5 has an immunization record on file that was last updated on 8/26/16.

Plan of Correction: I will request a new immunization record from the parents.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas and furnishing of the family day home, were not maintained in a safe and operable condition. Evidence: The plastic lining covering the refrigerator door in the programming area is peeling off, and could become a chocking hazard.

Plan of Correction: I will peel the plastic off today.

Standard #: 22VAC40-111-480-D
Description: Based on observation of the feeding area it was determined that not all children in feeding tables were using safety straps that were securely fastened. Evidence: Two children that were sitting at the infant feeding table were not using the chair's safety straps.

Plan of Correction: We will make sure we strap all children when using the feeding table.
