Seven children were in care total of 13 points. The children were observed during free play, lunch and rest time. There were two assistant present when LI arrived the assistant could not communicate with the LI . Provider arrived about 12:30. The provider stated she left at 9:00am. No children were on medications. Inspection time 10:30am-1:45pm

Standard #: 22VAC40-111-120-A
Description: Based on interviews and observation not all assistant's had a care giver record. Evidence: when LI arrived there were two assistants working with the children, only one had a record. The provider was not present for the first 1 1/2 of the inspection.

Plan of Correction: The assistant will not be working in the family day home as of today.

Standard #: 22VAC40-111-50-A
Description: Based on review of the written attendance record was not completed each day. Evidence: The last time the daily attendance was completed was 2/28/18.

Plan of Correction: I will keep a record of the children each day.

Standard #: 22VAC40-111-60-A
Description: Based on review of five children records not all were complete. Evidence: Child A's files was missing the start date, Mother and Father's work address and phone number, two emergency contact. Child B's file was missing the mothers work address and phone number.

Plan of Correction: I will ask he parents for the missing information.

Standard #: 22VAC40-111-130
Description: Based on interviews with caregivers not all were able to speak in English. Evidence: When LI asked Assistant A questions in English, the assistant could not answer the LI. The Assistant was caring for the children alone, due to provider not being present in the home.

Plan of Correction: I will only hire assistance that speak in English.
