22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced renewal inspection was initiated today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected on-site today between 2:00pm and 3:15pm. There were 5 children (plus the Provider's daughter), totaling 12 points, under the supervision of the Provider. Today, the children participated in circle time (learning ABC's and colors), free play, listened to storytime, ate lunch and rested at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children were served fish sticks, tater tots, green beans and milk for lunch. The Provider is not currently certified in MAT (medication administration training) and there are no children with any medication needs. Standard #: 22VAC40-111-90-C Description: Based on file review, 1 out of 5 children's records does not have documentation of additional immunizations once every six months up to the age of 2 years. Evidence 1. Child #4 is 2 years old. The most recent immunization record on file was completed when the child was 14 months old. Plan of Correction: The Provider will ask the parent for the most recent immunization record.
