22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted today from 11:30pm-1:30pm. At the time of inspection, 9 children (26 points) were directly supervised by the provider and three assistants. The sample size consisted of 3 children records and 4 caregiver records. There were no medications on site today and the provider is MAT certified. Children were observed during lunch time (rice, meat, yellow beans, zucchini, tomato sauce, and milk) and preparing for nap time. Diapering and handwashing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.
Standard #: 22VAC40-111-800-B Description: Based on documentation review, the provider did not review the emergency plan at least annually. Evidence: The last documented review of the emergency plan was in January 2016. Plan of Correction: Corrected at the time of inspection.
Standard #: 22VAC40-111-60-A Description: Based on review of three children?s records, the provider did not maintain complete children?s records. Evidence: The records for child #1 and #3 were missing the policy number for the child's medical insurance. Plan of Correction: I will get this information from the parents.
