Comments:
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:45-12 there were 4 children with the Provider for a total of 12 points within the supervision guidelines. The children were observed having morning free play, painting, riding a indoor horse, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance, and emergency procedures. Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, a child was observed drinking while walking. Evidence: Upon arrival, a child had a sippy cup with water in the hand while walking/playing in childcare area.

Plan of Correction: Provider immediately had child sit at table.

Standard #: 22VAC40-111-430-B
Description: Based on observation of physical plant, the provider did not have safety gate secured to prevent access to stair opening. Evidence: Upon arrival, the gate in the kitchen to prevent access to the basement stairs were found not securely fastened and the basement door with access to the stairs was found open.

Plan of Correction: Provider immediately closed the gate and the basement door. Provider stated that she opened the door to let the inspector in and forgot to close the gate. Provider will make sure the gate is closed and basement door is closed.
