Technical Assistance:
Reminded provider of background checks and sworn statements are now current for five years. Discussed qualifications for a substitute provider.

An unannounced Renewal inspection was conducted this date. A total of 12 children (28pts) with Provider and 2 assistants present today. When the inspector arrived, the children were divided into three groups. One group was observed completing a St. Patrick's Day art activity which involved coloring and gluing green glitter onto paper. A second group were seen playing with either blocks, puzzles or shapes that linked together. A third group which consisted of the younger children were playing freely with toys or climbing on the padded shapes that were available to them. A nice rotation of the groups was observed so the children were able to participate with each activity. Observation of diapering and hand washing procedures was made. Evaluation of all sleeping areas used for children and walk thru of the entire home was conducted this date. A total of 5 children's records, all staff records and household records were reviewed. The children were divided into two groups for lunch. One group observed eating lunch which consisted of spaghetti today. Staff observed sitting with the children while they ate and assisting when needed. Thank you for your cooperation today. This inspection began at 10:10am and ended at 12:15pm. Description: Based on review of all staff records, annual training was not completed. Evidence: Staff #2, 3 and 4 has not received the required 16 hours of annual training.

Plan of Correction: All three staff will complete an online training for 10 hours of training and will contact Office for Children for additional training courses to equal the required 16 hours annually.
