22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted on 03/14/2018 from 12:20pm-1:30pm. At the time of inspectors' arrival there were 7 infant to preschool aged children in care totaling 23 points with the provider. Children were observed participating in nap, feeding, diapering and transitioning between activities. A sample of 5 children's records,1 provider record and 1 assistant record were reviewed. Medications are not being administered at this time. There have been no injury reports since the last inspection. Areas of noncompliance are detailed in the violation notice and were discussed with the provider at the conclusion of this inspection.
Standard #: 22VAC40-111-920 Description: Based on observation and interview it was determined the provider did not post the current menu for one-week period. Evidence: The provider reported the menu was not posted. Plan of Correction: The provider will post the menu.
Standard #: 22VAC40-111-60-B Description: Based on record review it was determined that the center did not ensure that each child had a record that contained all of the required components. Evidence: 1. The record for child #2(enrolled 3/13/18) did not contain proof of the child's age and identity. 2. The record for child#4(enrolled 1/24/18) did not contain the name and policy number of the child's medical insurance and documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. Plan of Correction: The provider will request this information from the parents at pick up.
Standard #: 22VAC40-111-90-C Description: Based on record review it was determined that the provider did not obtain documentation of additional immunizations for children once every six months for children under the age of two years. Evidence: The record for child #3 did not contain documentation of additional immunizations for children once every six months for children under the age of two years. The last documentation was dated 11/1/17. Plan of Correction: The provider will request updated documentation from the parent at pick up.
Standard #: 22VAC40-111-570-A Description: Based on observation it was determined the provider did not ensure that 16 points were not exceeded. Evidence: The provider was alone supervising 7 children for a total of 23 points from 12:20pm - 12:49pm. The provider reported that they were over points due to a scheduling mistake. Plan of Correction: The provider will ensure that the they do not exceed 16 points per caregiver.
