Description: Based on a review of the first aid kit and an interview with the applicant, it was determined that there were missing components to the first aid kit. Evidence: There was no instructional manual or triangular bandages available.

Plan of Correction: We will get these items.

Description: Based on an interview with the applicant, it was determined that there was no battery operated flashlight or working portable battery operated weather band radio available.

Plan of Correction: I will obtain the battery operated flashlight and the weather band radio.

Description: Based on an interview with the applicant and through observation, it was determined that required phone numbers were not posted. Evidence: The numbers for 911, poison control and the emergency backup person were not posted.

Plan of Correction: We will post these numbers.
