Discussed with the provider: License cannot be issued until the CPS check has been received.

An Initial Licensing Inspection was conducted by appointment with the provider from 11:30 - 1:00. There are no children in care at this time but the provider has a fully equipped day care area in the lower level of a three level townhouse. The provider will accept children in care from Birth - 12 years. Her hours will be Monday - Friday from 6:00 a,m, - 7:00 p.m. Day Care area is equipped with many supplies for all ages of children. Description: Based on interview with the provider, it was determined that there is no written emergency preparedness plan.

Plan of Correction: The provider will use the form that is on the website to write the Emergency Preparedness Plan.

Description: Based on observation, it was determined that the sandbox does not have a cover that can be used.

Plan of Correction: The provider will get a cover for the sandbox.

Description: Based on interview with the provider, it was determined that the Provider's Child Protective Services Central Registry Check has not been returned to her. She resubmitted it 3 weeks ago,

Plan of Correction: The provider will call the Office to check on the CPS check and will fax it to the Licensing Inpector.
