Technical Assistance:
During the inspection, the licensing representative discussed with provider updates to background check policies and procedures. Provider attended recent training and is on track with meeting new guidance by September 2018.

An unannounced monitoring inspection was conducted today the Family Day Home from 12:15pm-1:30pm. A total of 12 children (36 points) were observed in the care of 3 caregivers. Due to timing, the children were observed only during naptime. A total of 5 children's records, 3 caregiver records, emergency drills, and other documents were reviewed. Description: Based on review and provider statement, documentation was not maintained for monthly evacuation drills. Evidence: Provider stated that an emergency evacuation drill was conducted February 2018; however, it was not documented.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-120-B
Description: Based on a review of records, 1 caregiver record did not contain the required information. Evidence: The record for Caregiver #3 was missing documentation of two or more references before employment.

Plan of Correction: I will contact the references again.

Standard #: 22VAC40-111-90-C
Description: Based on a review of records, the record for 1 child under the age of 2 years old did not contain documentation of immunizations every six months. Evidence: The most recent immunization record for Child # 4 [13 months old] was obtained 4/29/2017.

Plan of Correction: I will inform the parent.

Standard #: 22VAC40-111-320-G
Description: Based on a review, an annual inspection for a wood burning fireplace was not conducted. Evidence: The most recent fireplace inspection was conducted 12/14/2016.

Plan of Correction: I will schedule the appointment.
