Technical Assistance:
Standard 22VAC 40-111-60.B.14 regarding documentation of the annual review of the child's emergency contact information as required by 22 VAC 40-111-780 B was discussed with the provider.

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 12:00 PM and departed at 1:15 PM. Four children, totaling eleven points, were in care with one caregiver. A sample size of four children's records and two caregiver/household records were reviewed. The fingerprinting check of a potential assistant was reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported. After lunch activities and preparation for nap time were observed. Due to the children in care, points, and one caregiver, only the ground floor of the family day home was inspected. An exit meeting was conducted with the provider prior to closure of the inspection.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
