54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted beginning at 11:30 and ending at 1:45. There were 8 children in care including three who live in the home. Children were observed playing with toys and finishing lunch. Reviewed children's files- see violation notice. Standard #: 22VAC40-111-50-A Description: Based on interview with the provider, it was determined that there was no attendance documented for today. Evidence: The provider uses a book to document attendance but it was not done today. Plan of Correction: Attendance will be done daily.
Standard #: 22VAC40-111-60-A Description: Based on a review of the records for 5 daycare children that are present today, it was determined that not all records are complete. Evidence: There is no file for Child #5 - There is a physical and a shot record; Children 1, 2, 3 and 4 do not have Parent Information and Agreement forms; Children #3 and #4 do not have Local emergency contacts: Plan of Correction: The provider will review all children's records and get all files and complete information.
