Technical Assistance:
It was discussed that a master list of all children with physician diagnosed food allergies, food sensitivities and dietary restrictions must be created and updated when needed. All caregivers must be aware of the list and the children on the list. If the list is posted, there must be written parental permission for it to be posted within view of others.

A Subsidy Health and Safety inspection was conducted on 04/03/2018 to review supplemental health and safety requirements. The inspector arrived at 10:45 AM and departed at 12:30 AM. A sample size of three caregiver/household record and five children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based upon review of two caregiver records and provider interview, the facility has not ensured that caregivers employed prior to the effective date of the subsidy agreement have completed the ten hour Virginia Pre-Service Training for Child Care Staff. Evidence: 1. The record provided for caregiver 2 did not include documentation of the Preservice Training. 2. The provider stated that she was not aware that the Preservice Training was required of caregivers other than herself.

Plan of Correction: The provider responded with the following: Caregiver 2 will obtain the Preservice Training.
