The Licensing Inspector for Michelle Borst family day home conducted an unannounced monitor inspection on 4/18/2018. Inspection started at 1:25 PM & concluded at 2:45 PM. There were no children present during today?s inspection. Records for staff/household members were reviewed. Provider does not administer medication and does not transport children. An exit interview including review of Risk Assessment & areas of non-compliance found were reviewed with the Provider on 4/18/2018. Standard #: 22VAC40-111-180-A Description: Based on record review, the Licensee failed to obtain a current report of Tuberculosis Screening form every two (2) years from the date of the most recent screening. EVIDENCE: Staff #1 and Household Member #2 have not updated the required Tuberculosis Screening within the past two (2) years of the most recent screening. TB skin test/screening for Staff #1 was previously completed on 3/18/2016 and the TB skin test/screening for Household Member #2 was previously completed on 3/18/2016. Plan of Correction: Provider will update/complete TB skin test/screening and maintain documentation of the results in the staff/household records.
Standard #: 22VAC40-111-460-C Description: Based on record review & discussion with the Provider, the Licensee failed to ensure household pets are vaccinated for rabies and are treated for fleas, ticks or worms as needed. EVIDENCE: Household pet (Prissy) has not received vaccinations. Plan of Correction: Provider will return to the veterinarian to complete vaccinations.
