Description: Based on observation, the provider did not ensure a working battery-operated weather band radio was present in the home. Evidence: During the inspection, the licensing inspector asked to see the battery-operated weather band radio. The radio that was present in the home was not a weather band radio.

Plan of Correction: I will buy a weather band radio.

Description: Based on observation, the provider did not ensure stationary outdoor playground equipment was not placed over a hard surface. Evidence: During the inspection of the outdoor play area, the licensing inspector observed the end of the two slide chutes and the swings located on the outdoor playground equipment were over dirt. Grass was also around the playground equipment but the grass had been worn away at the end of the slide chutes and underneath the swings.

Plan of Correction: I will obtain material to place under the playground equipment.
