Technical Assistance:
Discussed new background check requirements for obtaining a fingerprint criminal record check. Suggested repairs to swing seats and climber to avoid cracked plastic from becoming a safety hazard

An unannounced monitoring inspection was conducted on 04/19/2018 from 11:36am - 1:30pm. There were six children in care with two caregivers. The ages of the children counted for a total of 15 points. Children were observed napping and playing games with the assistant in the child care room. Records were reviewed for six children and two caregivers. The inspector additionally reviewed written emergency procedures, the posted menu, hand washing, diapering procedures, and documentation of emergency practice drills. *No medication is being administered. The violation is listed on the violation notice and was reviewed with the provider at the conclusion of the inspection.

Description: Based on record review, the provider failed to obtain an annual review with the parent the emergency contact information and obtain the parent's signed acknowledgment of the review. Evidence: 1. The record for child 1 was last reviewed by the parent on 12/01/2016. 2. The record for child 2 was last reviewed by the parent on 11/30/2016. 3. The record for child 3 was last reviewed by the parent on 01/05/2017. 4. The provider confirmed the above information as the most recent review and update by each parent.

Plan of Correction: Each parent will be asked to review their child's record at pick-up this afternoon. The parent will be asked to sign the record verifying the review.
