Comments:
The licensing inspector conducted an unannounced monitoring inspection on Friday, April 13, 2018 from 1:50pm to 4:50pm. The children were observed resting upon the inspector?s arrival, and they were later observed eating snack. Snack today was ice cream and graham crackers. The menu was posted. Today, there were three children, totaling seven points, present with one caregiver.

The caregiver was observed interacting with the children, preparing their snack and guiding them through the routine of the day.

Four children?s records, and two caregivers? records were reviewed during the inspection.

Medication is not being administered.

The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There were repeat violations found during today?s inspection.

Standard #: 22VAC40-111-120-A
Description: Based on a review of caregiver records on 04/13/2018, the provider did not ensure that a record for each caregiver was maintained. EVIDENCE: The record for Caregiver #1 and Caregiver #2 were not available at the inspection.

Plan of Correction: Will pull everything together and have a complete file within 30 days. Will check files periodically to ensure they are complete.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records on 04/13/2018, the provider did not ensure that two of the four records reviewed contained the required information. EVIDENCE: 1. The record of Child #3 did not contain documentation of name and policy number of the child's medical insurance. 2. The record of Child #1 did not contain documentation of the policy number of the child's medical insurance.

Plan of Correction: Will get insurance information today.

Standard #: 22VAC40-111-90-C
Description: Based on a review of children's records on 04/13/2018, the provider did not ensure that documentation of additional immunizations once every six months for children under the age of two years old was obtained. EVIDENCE: The most recent documentation of immunizations in the record of Child #3, who is 23 months old, is dated 08/08/2017.

Plan of Correction: Will get additional immunizations by 04/20/18

Standard #: 22VAC40-111-140-B-3
Description: Based on a review of caregiver records on 04/13/2018, the provider did not ensure that a provider had current certification in cardiopulmonary resuscitation (CPR). EVIDENCE: Caregiver #1's CPR certification expired 03/2018.

Plan of Correction: Will have within 30 days.

Standard #: 22VAC40-111-530-A
Description: Based on observation on 04/13/2018, the provider did not ensure that rest mats and beds when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. EVIDENCE: 1. Two children were observed resting on cots without a top and bottom cover. 2. One child was observed resting on a bed without a top cover.

Plan of Correction: Corrected on site.

Description: Based on a review of caregiver records on 04/13/2018, the provider did not ensure that each caregiver record contained documentation of a sworn statement or affirmation and search of central registry. EVIDENCE: Caregiver #1 did not have a record available to review at the inspection.
