An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 6 children, totaling 16 points. Children's records, caregiver records, training documentation, children's nutrition, emergency preparedness items, and the physical plant were inspected today. The children were observed gluing art projects, eating lunch, assembling puzzles, and playing with toys. The children were served chicken burritos, bananas, green beans, and milk for lunch today. This inspection was completed between approximately 9:45am and 11:35am. Thank you for meeting with me today. Standard #: 22VAC40-111-940-A Description: Based on observations, it was determined that the downstairs refrigerator has not been maintained at 40degrees or lower. Evidence: The thermometer inside the children's basement refrigerator indicated a temperature of 50degrees. Plan of Correction: I think the thermometer is broken because the food stays very cold, and sometimes freezes in there. I will buy a new thermometer and monitoring the temperature.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that two records were not complete. Evidence: Assistant record A does not include emergency contact information. Assistant record B does not include emergency contact information and a second reference check. Plan of Correction: Both files will be updated today.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #1 does not include an annual review completed by the parent. Child record #3 does not include the father's employment contact information. Plan of Correction: I texted both parents during the inspection.
Standard #: 22VAC40-111-70-A Description: Based on a review of administrative documents, it was determined that two parents have not been given all written information as required. Evidence: Child record #3 does not include a signed "Provisions of the Emergency Plan" document. Child record #4 does not include a signed "Liability Insurance" document and "Medication Policy" information. Plan of Correction: I will ask both parents to sign the forms today.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver health records, it was determined that two TB Health Screenings have not been repeated within the past two years. Evidence: The TB screening for the provider expired on 8/31/17. The TB screening for assistant caregiver B expired on 9/14/17. Plan of Correction: We will both get a new TB screening done.
Standard #: 22VAC40-111-250-E Description: Based on observations, it was determined that balloons were accessible in the child care area. Evidence: Upon arrival, the Inspector observed eight balloons on the floor and four additional balloons hanging on the wall. Some of the children were observed playing with the balloons on the floor. Plan of Correction: The balloons were immediately popped and thrown away.
Standard #: 22VAC40-111-320-B Description: Based on observations, it was determined that various electrical outlets were not covered. Evidence: Four electrical outlets were not covered. In addition, the Inspector observed two different children pulling an electrical outlet cover off of the wall outlet. Plan of Correction: All outlets will be kept covered. I will buy new, tighter outlet covers.
