As discussed, your mini trampoline must not be used for the children in care.
An unannounced monitoring inspection was conducted this date. A total of 12 children (31pts) with Provider and 2 assistants were present during the inspection. When the inspector arrived, the children were completing circle time, singing with staff to nursery rhymes. The younger children were being held by staff, waking up from morning naps and having diaper changes. Children were observed participating in gross motor activities by dancing to music and singing. They transitioned smoothly to lunch time. Children were all seated at the tables. Lunch today was pasta, chicken, veggies and apple sauce with milk to drink. Children participated in bringing their cots out for nap time and laid quietly and were observed falling to sleep. A total of 5 children's records and all staff records were reviewed. Emergency evacuation drills reviewed. No medications on site. All topicals were inspected and found current and labeled. Thank you for your cooperation today. Standard #: 22VAC40-111-100-A Description: Based on child record review, a health exam for one child was not obtained within 30 days of care. Evidence: Child #4 began care on 11/27/18. Health exam was not obtained until 3/2018. Plan of Correction: I will remain all parents that health exams are required within 30 days of beginning care.
Standard #: 22VAC40-111-90-A Description: Based on child record review, immunizations were not obtained for one child prior to beginning care. Evidence: Child #4 began care on 11/27/18. Immunizations were not obtained until 3/2018. Immunizations are required prior to enrollment. Plan of Correction: I will remind all parents that immunizations are required before they start care.
Standard #: 22VAC40-111-170-A Description: Based on staff record review, a TB screening was not obtained for one staff. Evidence: Staff #3 did not have a TB risk assessment/screening on file. Plan of Correction: This staff has had the TB screening and is waiting for the doctor to complete the form. We will obtain the form and maintain in the record.
Standard #: 22VAC40-111-180-A Description: Based on staff record review, one staff did not have a subsequent TB screening. Evidence: Staff #1's last TB screening is dated 1/2016. A TB screening is required every two years. Plan of Correction: This staff will obtain a TB screening this month.
Standard #: 22VAC40-111-280 Description: Based on observation of the physical plant, one hazardous substance was found unlocked and accessible to children in care. Evidence: Clorox wipes was found under the bathroom sink cabinet which was accessible to the children in care. Plan of Correction: We immediately secured the clorox wipes.
Standard #: 22VAC40-111-680-B Description: Based on observation of the programming, staff did not ensure that hands were washed as required. Evidence: Staff did not ensure that one child washed hands after using the bathroom. Staff did not ensure that all children's hands were washed prior to eating lunch. Plan of Correction: We will review hand washing requirements with all staff and ensure that hands are washed as required.
