Technical Assistance:
New fingerprint criminal record check procedures reviewed. The provider has stated the outdoor climber is not in use at this time.

An unannounced monitoring inspection was conducted on 04/20/2018 from 10:20am - 12:10pm. There were six children in care with two caregivers. The ages of the children counted for a total of 15 points. Children were observed playing with toys and game with both caregivers in the child care room. Morning snack consisting of fresh oranges, cheese, tortilla chips and water was served. Records were reviewed for the six children in care and both caregivers. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Standard #: 22VAC40-111-830-C
Description: Based on review and interview, the provider failed to ensure that documentation shall be maintained of emergency evacuation fire drills. Evidence: 1. The emergency log for the family day home does not have documentation of a fire drill for February and March of 2018. 2. The provider stated both drills had been completed but not documented on the practice log.

Plan of Correction: A new emergency log is now in place. All emergency drills will be documented to include February and March.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview, the provider failed to ensure that each caregiver shall obtain a current tuberculosis screening every two years from the date of the first screening. Evidence: 1. The most recent tuberculosis for caregiver 1 was conducted on 03/18/2016. 2. Caregiver 1 confirmed this was the most recent TB screening.

Plan of Correction: Caregiver 1 has an appointment scheduled in May. The TB screening will be updated at this time.
