The provider had questions about the background check requirements. The background check requirements were reviewed thoroughly including fingerprinting requirements.
An unannounced renewal inspection for the provisional license was completed on 4/24/18. There were no enrolled children in care; however, the provider's infant grandchild was visiting. The provider and one household member was present. Four children were present for care before school and two up to four children were expected to be in care for after school care at 4:00 pm. Six children's records were reviewed for updates only. No new children were enrolled. The provider's record and two household member's records were reviewed. The provider does not administer medications. The inspector arrived at the provider's home at 11:00 am and departed at 2:45 pm. The inspector reviewed children and other records, observed the areas of the family day home, reviewed the attendance records, observed the required posted items, reviewed emergency drills and procedures, observed emergency supplies, reviewed the provider's annual training, and reviewed the items required for transportation. There was discussion with the provider about the annual review of the emergency plan due in June 2018. The provider will submit a renewal application as soon as possible. The new application was reviewed with the provider.
Standard #: 22VAC40-111-730-D Description: Based on observation, the provider failed to ensure that all medications were kept in a locked place using a safe locking method to prevent access by children. Evidence: 1. The provider's first aid kit was observed to have several "sample" packets of medication (non-aspirin and acetaminophen). The first aid kit was not kept in a locked place. The first aid kit was stored in an unlocked pantry closet off the kitchen on a shelf approximately four feet tall. Plan of Correction: This was corrected during the inspection. The provider removed all of the sample packs and put them with the household medications that are locked.
Standard #: 22VAC40-111-180-A Description: Based on record review and interview, the provider failed to ensure that each adult household member had an updated tuberculosis (TB) test or screening within two years from the date of the last test or screening. Evidence: 1. The last completed TB test for an adult household member was 3/31/16. A test or screening would have been required to be completed by 3/31/18. 2. According to the provider, the household member had not had an updated test or screening. Plan of Correction: The provider stated that the household member would be going today to get a TB test completed.
