Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 26, 2018 from 12:00 p.m. through 2:00 p.m. There were five children present along with the provider for a total of fourteen points. Five children's records were reviewed. The provider's record and the record for three household members were reviewed and updated. No medication is being administered at this time. Areas observed were lunch preparation, lunch, free choice play, and nap. Interactions between the provider and children were observed as positive and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-111-60-B Description: Based on record review the provider failed to have the physician's contact information and a second emergency contact for one child. Evidence: 1. Five children's records were reviewed. 2. Child #1 did not have documentation of the physician's name, address and telephone number. 3. Child #1 did not have documentation of a second emergency contact's name, address and telephone number. 4. The provider confirmed this documentation was missing. Plan of Correction: I will have the parent provide this information.
Standard #: 22VAC40-111-680-B Description: Based on observation, the provider failed to have the children wash their hands before eating lunch. Evidence: 1. Lunch was served to the five children in care. 2. Five children were observed not washing their hands before eating lunch. 3. The provider confirmed the children did not wash hands before eating lunch. Plan of Correction: I will remember to have the children was their hands before eating.
