Discussed License and terms of license as it pertains to risk when not in compliance. Discussed Assistant's child- that child counts in capacity and the points.
An unannounced inspection was conducted from 930-1030 AM. The Inspector observed 8 children ( 2years- 5 years) with the Provider for a total of 14 points. The Provider stated two children present ( #5 and #6) usually come in the afternoon but just came in today. The attendance record documented that #5 and #6 have come in the afternoon and the Provider stated Child # 3 and #7 leave before the afternoon children arrive. The Provider needs to send a letter requesting a capacity modification of 7 children. The Inspector reviewed all 8 children's records, emergency drills, menu, and observed program. The Provider stated there was no one present today that is on medication. Please call with questions 703-479-4679
Standard #: 22VAC40-111-30-A Description: Based on observation and interviews, the Provider is not ensuring capacity compliance with the standards and the terms of her current Conditional License. Evidence: 1. The Inspector arrived and observed 8 children present with the Provider. The license has a capacity of 6. 2. The Provider stated she was over but that zoning allowed for 7. Plan of Correction: I will make sure that the numbers stay and that children do not overlap since they are part time.
Standard #: 22VAC40-111-60-B Description: Based on review of 8 children's records, the Provider does not have complete records for 4 children. Evidence: 1. Child # 4 has written Information for parents but the parent has not signed the form. 2. Child #6 does not have a start date documented but the parent signed the enrollment information form on February 22, 2018. 3. Child #7 does not have a recorded start date but the parent signed the enrollment form March 22, 2018. 4. Child #8 has only one of two required emergency contacts listed on the enrollment form. Plan of Correction: I will have parents fill out.
